Health Insurance Contribution Assistance (PBI JK) membership for under-five children is the lowest compared to other age groups. Their membership is essential to improve health protection and utilization, especially among low-income families. Through Government Regulation 76/2012 and Presidential Decree 82/2018, under-five children whose mothers are members of PBI JK can automatically become members. This article aims to provide recommendations to increase PBI JK membership for children under five. This study employs mixed methods, including in-depth interviews with 67 stakeholders and 32 PBI JK recipients, analyzing Susenas and Podes Data of 2021. The study was conducted in three districts, i.e., Cianjur, Banyuwangi and Manggarai. The analytical framework follows the 4P approach (Program, Process, People, Performance). This study found that three aspects can influence the low membership of PBI JK for the under-five group: (i) Program, the 3 months time limit to register a newborn as a member of PBI JK is considered too brief, especially for those who live far from public services; (ii) Process, the procedure to obtain required document and to register is still ineffective, potentially burdens the members and the staffs; PBI JK’s mother more likely to deliver out of the health facility that provides civil registration services; (iii) People, there is limited understanding about the importance of membership in PBI JK. Social, economic and cultural aspects sometimes affect the program’s performance.
